Your First Visit

On your first visit, you will be asked to fill out a general health history questionnaire that also includes a Consent to Care paragraph and Cancellation / No Show policy. Depending on the reason for your visit, you may be asked to fill out further papers related to auto insurance, accident information, etc.

Please arrive at least 10 minutes early for your first appointment so that you will have enough time to fill out the health questionnaire and so that your massage can be started on time without losing time on the table.

If our office door is closed on your arrival Melissa is mostly likely with another client or returning phone calls and will be with you shortly.
